Cost of Future Care Evaluation (CFC)

For individuals with permanent, functionally limiting chronic health conditions.

Testing determines functional status and areas where items and assistance is required.

Categories considered include: treatment and health maintenance, equipment and adaptive aids, home support services (e.g. housekeeping), and future surgeries. Some individuals have a few areas of need and others have many.
Functional Abilities Evaluation (FAE)

A half-day functional evaluation that focuses on functional status.

Next step recommendations are provided to maintain/improve daily functioning.
Cognitive Functional Capacity Evaluation (CogFCE)

Multi-day testing protocol that is an extension of the Standard FCE but also includes functional cognitive testing to assess impact on work and daily functioning.
Life Care Plan (LCP)

A more extensive CFC report designed for individuals with complex and changing needs throughout their lifespan.

Functional assessment, as well as interviews with family and treatment providers are often conducted.

The report serves as a blueprint for the future to promote maintenance and stability of the individual’s condition.
Training and Education for Clinics and Practicing Clinicians

Clinics or individual clinicians are taught evidence-based tests and protocols for their practice. Workshops are planned to meet specific educational needs.

Consultation re: challenging clinical issues.

Education on effective court testimony or case specific court preparation.

Review of business practice from referral intake to report delivery to maximize efficiency.
